Vaccine Requirements

Prepared for patients and caregivers that require an evaluation of due immunizations for a specific age or comorbidity state at the current time. We evaluate the immunization history and the clinical notes provided against the most up-to-date CDC guidelines to make a recommendation regarding due vaccinations or boosters. An up to 1000 words consulting report will be shared with the patient and/or caregiver within 3-5 business days after complete documentation and payment have been received. This includes an assessment of the health risks associated with not pursuing any recommended immunizations. While a video counseling session is not available for this package, one 10 min Q&A session is included for up to three months following the initial consult request.
